Thank you for your interest in volunteering with the Municipality of West Perth.

• If you are participating in programs which include being exposed to young children and youth. This requires you to provide a proof of a Criminal Background Check.
• You may be required to volunteer in indoor settings like the West Perth Youth Centre, the West Perth Community Centre, and Commercial Kitchen, or in outdoor settings such as parks, trails, pavilion spaces, and recreation fields.
• You may encounter different risks throughout your time volunteering. These risks could include exposure to different food or airborne allergens. If you require medical aid devices (e.g., Epi-Pen), you must let the staff supervisor know.
• You will follow all safety protocols deemed essential and mandatory by the Municipality of West Perth.

Volunteers recognize the potential for injuries to themselves and participants, as well as their physical and personal property which may result from volunteer activities conducted with the Municipality of West Perth. The Municipality of West Perth and all MWP partners intend those volunteers expressly assume all risks and liability for any injuries to, or caused by, volunteers.

I understand that during my participation in volunteer activities I may be exposed to a variety of hazards and risks, foreseen or unforeseen. These include, but are not limited to, damage to property, personal injury, illness, or death during volunteer activities. I understand that at volunteer activities or events, there may not be access to medical facilities or anyone with the expertise necessary to deal with injuries and damages to which I may be exposed.

Programs put on by the Municipality of West Perth intend those volunteers participating in any activity are not acting on behalf of the MWP or any partner in any capacity. As such, it is the MWP’s intent that volunteers are not authorized to be considered agents, employees, or authorized representatives of the MWP or MWP partner for any reason.

In consideration of the foregoing, I, myself, my heirs, and executors so hereby release and discharge the Municipality of West Perth and all of the MWP’s partner agencies for all claims, damages, actions, and whatever in any matter arising or growing out of my participation in activities sponsored or organized by the Municipality of West Perth.

I acknowledge that, in the course of performing and fulfilling my duties as a volunteer, I may have access to and be entrusted with confidential information concerning the present and future activities of the Municipality of West Perth, the disclosure of any of which confidential information is prohibited.

I further acknowledge and agree that the right to maintain the confidentiality of such information constitutes a proprietary right which the Municipality of West Perth is entitled to protect. Accordingly, I agree that I will hold in strict confidence, except as required by law, all information concerning the business and affairs of the Municipality, its clients, personnel, volunteers, partners, and donors acquired in the course of the professional relationship, and not use this information for personal gain.

I, the undersigned, have read and understand the Information for Volunteers above, and agree to the Municipality of West Perth’s Liability Release, Code of Ethical Practice, and Confidentiality Agreement terms here within.

I hereby give absolute right and permission to the Municipality of West Perth to use my name, and to publish, copyright, and use images of me in which I may be included in whole or in part.

Such publication many include print, broadcast, web, video, or other social or electronic media.

I grant permission to the Municipality of West Perth to permit use of these photos by third parties. I also provide permission for the photo to be stored in a photo bank for future use.

Use of this photo includes promotion of the Municipality of West Perth and their work and programs, and or the work of partner organizations and or government ministries and agencies and may include public and media relations, educational videos, fundraising, programs, and projects or other promotion.

I understand I will not receive any payment for the use of the images and or videos in which I appear.

If the person photographed is under the age of 18, I certify that I am their parent or legal guardian and I grant absolute right and permission for use of their name and image.
